When you have a safety officer, the officer must be documented as the safety officer, coming from the CEO as an appointment of the position.  This document can be purchased, which has the template; you only need to fill in the blanks to make it official.  This appointment template also has the duties of the safety officer outlined for their ease of understanding the magnitude of the appointment.
